Case Management

A case manager monitors patients in all neuro units. Their focus is on facilitating transitions through discharge to an appropriate setting in a timely manner. They facilitate a multidisciplinary approach to discharge planning and ensure that patients have the equipment needed whether they return home, transfer to a rehabilitation facility or are placed at a long-term care facility.

Ambassador-level Care
Our Patient Care Ambassador provides individualized, one-on-one customer service to patients and their families. The ambassador extends the reach of the care team and make each patient feel welcome, comfortable and important. The ambassador provides the extra touch that makes a difference in the patients stay. This may range from providing reading materials to make the patients’ time pass more quickly, or to being a good listener during times of stress. The ambassador works closely with the manager and nursing staff to inform them of concerns in an effort to avoid problems.

Pastoral Care
Our patients have the option to receive visits from a pastoral care representative. Pastoral care offers comfort, prayer, counseling and spiritual practices and rituals. This member of the team provides spiritual support for patients and their families during their stay in the hospital.
